Person-Centred Care

The Person-Centred Care Program is based on an approach to care that involves partnering with patients and healthcare providers. This approach gives patients a voice in the design and delivery of the care they receive and enables them to be more active in their experience. The intended results include:

  • better health outcomes
  • greater value through wiser use of resources
  • improved  experience for patients, families and caregivers
  • improved quality and safety for patients and providers

Person-centred care is the evolution of patient-centred care, a shift that signals the importance of being treated as a person first, and as a patient second. The term “person” also includes family members and caregivers.

Goals

  • Actively partner with patients to identify, design, plan and improve healthcare services
  • Develop and implement programs and resources that drive the adoption of person-centred care in service delivery

Key Initiatives

  • Launched the Person-Centred Care Guideline and accompanying video
  • Developed the CCO Patient and Family Engagement Report
  • Launched the Patient and Family Advisory Council Toolkit
  • Developed the provincial plan, Quality Person-Centred Systemic Treatment in Ontario, in partnership with the Systemic Treatment Program
  • Developed the strategy, Navigating the Diagnostic Phase of Cancer: Ontario’s Strategic Directions, in partnership with the Diagnostic Assessment Program
